A fault reported by phone has one property: it exists only in the head of whoever answered. A written report with a photo and a timestamp can be passed on, found again and proved.

Why three fields and not a page of them
The report is written standing next to a machine, often one handed. Every extra field means the fault gets phoned in, or not reported at all until the shift changes.
The photo is the most valuable part and on a phone it is free: the button opens the camera. From it the manager can tell whether to go now or wait for planned downtime.
Time and place fill themselves in. A form should never ask for something it already has.
When it makes no sense
When nobody reads the reports. A form that emails an inbox nobody opens is worse than a phone: people learn that nothing happens.
And for emergencies. Those need a phone number in large type, not a form.
